Abstract: This project will apply the program using tailored Buffalo Functional Exercise to 200 community-dwelling older adults who have the highest risk of losing independence in Rochester Medicare enrollees. The main purpose is to quantify the effects of the one-year program in a reduction in hospitalization and medical expenditure due to fall injuries and all-causes. This quasi-experimental study with a PT intervention and bi-weekly reminders will compare the results with those of previous records, using Chweicko’s Per Member Per Month variance reduction method.
